Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed on the same day as the consultation appointment as long as there are no contraindications to treatment and the treatment is performed using local anesthesia. However, a complex medical history or complex treatment plan will require an evaluation at your first visit and a second appointment for surgery.
At the initial appointment, Drs. Fraser and Michaels will discuss with you the procedure they will perform, explain any risks or possible complications with the procedure, and answer all questions you my have. It is our intention to help you understand your anticipated surgery and alleviate any preoperative anxiety you might have.

For your convenience, you may complete the Patient Registration form and review our Privacy Policy online. Please click on the link to submit your Patient Registration and also click on the link to print the Privacy Policy Acknowledgment form. The acknowledgment form should be signed and returned to our office at your initial visit.
IMPORTANT: All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (heart medications, aspirin, blood thinners, etc.).
If your dentist or physician has taken x-rays, you should request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office at an additional charge. Teeth displaced within the jaw are difficult to examine with conventional dental x rays. We often use a 3-D digital radiograph to provide an overall view of both the upper and lower jaws and the relationship of the teeth to the adjacent blood vessel or nerve, as well as the sinus cavity. We do accept Panorex x-rays taken by your general dentist if they are of acceptable quality and have been taken within 1 year of your surgery.
