Consultation evaluation fees are required to be paid in full at the time of service, if you do not have insurance coverage. If you have insurance, we will file your insurance claim as a courtesy to you and no down payment is required for your consultation appointment. For your surgery, we will ask that you place a percentage down on the day of the procedure. The percentage amount required will be dependent upon your insurance policy’s annual deductible and yearly maximum allowable amount. We will file your insurance claim immediately following your procedure and any balance remaining that is not covered by your insurance company within 60 days will be your responsibility to pay in full. If your insurance company pays more than we estimate, we will refund you the credit amount.
For your convenience, we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to you and payment is due at the time service is rendered unless other arrangements have been made in advance. Most insurance companies will respond within four to six weeks after your claim is submitted. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. We will send you a statement. Your prompt remittance is appreciated. Information is available for a monthly payment plan with Care Credit, an independent financial institution, pending your approval. Care Credit should be implemented prior to your actual surgical procedure.
Care Credit is a credit card exclusive for health care services. It is a no interest or low interest financing option each time you use it. You can use your card as often as needed for additional procedures as long as you remain below your credit limit. The card may also be used at other health care provider offices as long as they are registered with Care Credit.
